Check in to visit my mom was pleasant. Receptionist very polite and professional. My biggest shout out early in this transition for my mom is for Patrice. She has extremely kind, caring and helpful through this new journey. On my first visit she had my mom eating a late lunch in a wheelchair down by the nurses station. It appeared to be a full regular hot lunch meal. That showed caring to me. Will meet staff Monday I hope for a Care Plan meeting. Thus far, I’m very pleased with the facility.

Just like most of these places, there are pros and cons. There are some great staff there, especially Rebeca in speach therapy. She got my uncle eating again after his stroke. She is a rock star. Unfortunely, the cons outweigh the pros. My uncle fell out of bed 3 times. I have a picture of the gash on his head and they didn't even clean up the blood from his face and shirt hours later. I advised that they should put pads on the floor but they said they wouldn't do it because it is a tripping hazzard. My uncles leg is paralyzed so there is no possibility of tripping. And they don't have guardrails because of other liabilities. My uncle also developed a bed sore on his shoulder. For as much as these places charge, they should be able to do better. The front door and electrical system is wonky. There are potholes all over the parking lot. My uncle was there for 4 months. When he was discharged, Chastity the social worker was supposed to help with ordering equipment and supporting his venture home. Not only is she very unfriendly, but her lack of timely and accurate communication has resulted in us still not having a hoyer lift or wheelchair at home....a month later! So my uncle has been in bed for a month. I'm disguested by the elder care in this country and my guess is that it's hard to find a great place these days in corporate America.

Visiting a friend and we enjoyed a conversation in the cafeteria. Very clean and bright area to share some time together. Marlene and other staff stopped briefly to engage in the conversation and laughter. So happy my friend is a part of this agency. ?

My husband was just released from Creekside on Monday, memorial day, and was already rushed by our doctor to the ER yesterday/Thursday. He had very severe edema in his legs and feet and had fallen. He also was having some fluid buildup in his lungs. The high sodium diet at the facility and sitting for long periods of time without his legs elevated caused the major fluid buildup. The food at the facility was nothing like you would expect from a place that is supposedly rehabbing people!! There was never any fresh fruits or fresh vegetables, the entire time!! So I was taking in salads, fruits and vegetables, so my husband could get something nourishing and some fiber.

Several members of the staff, the CNA’s, and the therapists were very caring and helpful, but they were understaffed and can only do so much! The patient rooms need some major renovations, and a thorough cleaning!!

So once the excess fluid is removed from my husband‘s legs, and he’s discharged from the hospital, he will need to go back to rehab, but it will not be Creekside! Never again would I take him there!!!

I’ve only been here for three days, but from what I’ve seen, it is good! Although I would rather see if it is possible to have bars on both sides of Mr. Camps bed. He is moving around a lot so I thing it is very possible for him to fall off the bed or worse. Please be advised of this possibility! And treat it as a hazard!!!! Thank You… BobbyC.

Update from previous post: since Creekside took over, everything went down hill. Food was nearly cut in half and a lot of times my husband never received his cranberry or apple juice drink for breakfast and lunch. There were a lot of things that went on-the CNA’s and nurses that I spoke highly of were never rewarded in addition to bonuses were taken away. Typical organization that only cares about their own pocketbooks. My husband passed in July, and sadly, the Director of the facility never called me or sent a note of condolences given the fact my husband was a Vietnam Vet. There is no phone number for corporate to speak to someone about issues. My loving husband was at this facility for 22 months and it was the best while Manor Care and Promedica was running the facility but not this organization.

Want to acknowledge Faye and Laverne Turner for their exceptional caring, thoughtfulness and expertise they show to my husband. David, the day nurse is very caring as well. If it wasn’t for Faye and Laverne, not sure my husband would be at Creekside. There are quite a few exceptional nurses and CNA’s. I realize there are times where agency staff is needed, but lately, there have been more than when Promedica was in charge. Giving 4 stars due to the influx of agency workers. Thank you.

I was so delighted by the help that Sandra C. provided. She was pleasant and kind to the residents as we walked through the facility. I met with Edwina, who was the nurse responsible for the resident. Edwina was very helpful and treated all with upmost respect.

This place is terrible. Do not send your loved ones there. They kept telling my loved one. It was in his head that his stomach was hurting trying to give him Tums. He has an infected gallbladder. He is so dehydrated because they do not Give them water but little dinky cups. They are not allowed to take into their bedrooms. This place is terrible. I am turning them in plus calling the news media. Oh yes, and let's add that the in-house doctor. Rate in front of my face face face-to-face told me his blood work was perfect. His kidney is functioning at 2.7%.
